Thomas McGath grew up in a fair household for his time. His mother was a nurse who caught the nasty tuberculosis that's been almost plaguing his hometown. She unfortunately passed away when he was 14. This explained his motivation to become a physician. At 18, a medical school accepted him, however it was not cheap. He did not qualify for any of the free programs available. Being so young and already in debt scared him. He felt that telling his father about it would cause disappointment and that it would scare him, too.

As if it couldn't get worse, after having finished medical school at 22, his childhood friend, Connor Harrison, asked him for help with something that wasn't exactly legal. He didn't know about this until a year later, when his friend admitted to smuggling drugs for cartels and was using Thomas as one of his suppliers. He was now asking Thomas to help him hide because he didn't want to be a part of the gang anymore. Not much longer, the Douglas Gang finds them both. On the way back to the gang's hideout, police recognized them and attempted to stop them. Some members, including his childhood friend, were arrested, however many were able to escape, but not without injuries.

Henry Douglas, the leader, ordered Thomas to help. It was the scariest day of his life, not knowing what would happen to him if he refused, or if one of the members died in his care. Everyone survived, however one would suffer a permanent injury for the rest of his life. For a while they didn't know what to do with the physician and he overheard a few arguments. Henry's mistress, Audrey, would visit him in secret, offering to secure his safety and that he would owe her a favor. At this point he hadn't eaten in almost a week and was rarely given water. She gave him the night to think about it. It was an offer that he couldn't refuse. When they met in the morning, he agreed.

Now he is 26 and has been a member of the Douglas gang for four years. Despite having recently become accustomed to being a gang member, Thomas has been planning an escape since the beginning, and the gang had only just started to trust him. Another reason he has been delaying his escape attempt is that they have been promising to help with his debt and they have been genuine about that. They have argued whether or not to make him sell snake oil and eventually settled on advertising their physician to wanted men and women who wouldn't trust others to report them to the police. He used to have to travel with other gang members, but recently he was able to convince Henry that him traveling alone would be less suspicious, especially since his public identity is not yet known to be associated with the Douglas gang.

Henry's mistress asked her owed favor to Thomas last week. She wants him to somehow complete this task without the rest of the gang's knowledge.
