There are quite a few scenes that are different in the series compared to the books. For example, the entire Farmhouse scene, with the stealing of the silver, and then finding the people dead, and Sandor digging them graves, is not in the books.
Okay, so I think I've narrowed it down to the following scene:

Except, of course, that Sandor's not a sworn knight.
But that definitely seems like it relates to clue 2, yes! So, now, what's up with the chimneys? Could that be a reference to the Twins, like was said earlier?

House Roote of Lord Harroway's Town blazons their arms with a two-headed horse, brown, on a field of wavy green and green.
The ferry in Harroway's Town has carved horse heads, and is I think referred to as "the two-headed horse"
The two headed horse is the boat from what I can find as well
