You grow tired of paying excess medical expenses, and on the time of a critical surgery, a friend pointed you towards an 'underground' doctor, who is willing to work on patients free of charge. People without money and people of criminal origin go there to get there wounds healed, and healed well those wounds are. So there you are in a shabby room, it smells of heavy sanitation with the light hint of blood among other bodily fluids. Suddenly, the door opened, and a tall lanky figure slipped in, its' head covered with a brown paper bag with only one red dot for an eye. "Come... we must operate now..."
On very, very, rare occasions there may be unexplained, POSSIBLY supernatural events. Minor "environmental" magic. Perhaps implies the existence of a deity or supernatural entity. Or perhaps it's just tricks?
Modern
A peaceful setting, but with a chance for small fights and struggles to break out on a very small scale.
Romantic themes are possible, but won't pan out. For example: unrequited love.
