Diagnosis:
Bipolar 1, with manic episodes. Major depressive disorder. Intense mood swings. Suicidal tendencies related to diagnosis. Hyper-sexual.

Addictions:
Sex, drugs, alcohol.

Medications:
Previously used lithium, and atypical antipsychotic medications. Patient refuses to take them on a regular basis and becomes irritated when the subject is brought up.

POLICE/INCIDENT REPORT

Mode of Admission:
Patient was taken to the hospital by way of ambulance, after being found wandering and singing/screaming in the streets, after escaping the last hospital. She is a frequent flier when it comes to the hospitals.

Mental State on Admission:
Confused, unable to speak or think clearly, occasional screaming, refuses to answer any questions.

Physical Condition on Admission:
Patient was dressed in a short, likely stolen, black cocktail dress, and she had blood on her hands and mouth. No shoes, or leggings to be found.

Doctor’s Notes:
“This patient can be highly dangerous, and highly unstable when unmedicated, and should not be left alone with any sort of sharp object. Treatment is ongoing, but patient makes this difficult by refusing to participate in any way, shape or form.”

Physical Description:
He stands at six feet, five inches and is built exactly like you think a Viking of old would look like. He has long blonde hair that he wears braided down his spine, is stops just below his ribcage. He has stormy-grey eyes that have a child-like look in them. His body is heavily scarred. His back is nothing but a tapestry of abuse and torture with lash and whip marks crisscrossing every inch of skin from the back of his neck to the top of his buttocks. His arms are scarred as well with what looks like cut marks from blades. His chest is littered with star-shaped scars that appear to be from ancient arrow heads.

Diagnosis:
Severely traumatized, PTSD, victim of rape, easily manipulated

Running - he has to run every day or he gets agitated

POLICE/INCIDENT REPORT
Meph, though his name was known to the authorities until recently, had open active and cold cases with numerous city, county, and state police departments. The FBI, CIA, and countless private detective agencies were also looking for this undetectable assassin. But he was a ghost. He existed nowhere on paper. His fingerprints weren’t in the system. No one had ever heard of him. Until 6 months ago...

Meph was shot five times in the chest by a rival mob-member when Meph showed up to murder ten of them. From what was found at the crime scene, Meph managed to massacre nine before the tenth shot him, severely wounding him. The bullets missed Meph’s heart but did manage to severe his aorta. Thanks to modern medicine, his death blow was anything but. He was picked up by an ambulance and taken to the hospital where a ten-hour surgery all but fixed him up.

When the nurses were going through his clothes, they found a note folded up in his wallet. A wallet that held no ID, no credit cards, no money, only this note.

'Mephistopheles Norwish, "Meph" for short. He's the deadliest creature ever to be made. Why is he the deadliest? He was made by two Alphas at the same time. He imbibed the strength of both of the alphas and their packs, so he cannot be controlled by any normal alpha. To make it worse, a few years back, another Alpha bit him and, in an attempt to attach him to her, imbibed her pack's power into him as well. No Alpha can control him. He kills brilliantly though. My WARNING to you is, if you don’t have jail cells or guards strong enough to hold him, keep him drugged, wounded, or both. Don't let him get to full strength or he'll tear a hole in whatever you keep him in. He doesn't know he's got all this power or what he's capable of. He has been kept in a cage and treated like he was nothing since he was a boy. If we hadn't, he would have killed us all.’ The nurses gave the note to the arresting officers, not sure what it meant. Meph was arrested and placed in a jail cell.

Doctor’s Notes:
Due to the amount of damage that was done to his body, obvious signs of torture, Meph was given multiple psychological evaluations by some of the most notable psychologists of the day. Every single one of them proclaimed Meph to be a victim of abuse to the point where he was unable to tell right from wrong. He knows nothing of good and evil. He will only answer directly asked questions and his answers are brief but honest. The psychologists shared their results with the courts which spared Meph from going to trial but caused him to be placed in a mental institution rather than prison for the remainder of his life.

5' 3'' tall female with indigo blue dyed hair that goes down her mid back. Her eyes are dark blue and they appear just dark when the light is dim, only showing their true colour under the right lighting. Her body is athletic, a result of intensive training also presenting feminine curves a narrow waist and wider hips. Her skin is slightly pale and she presents a cluster of scars across her upper back.

Diagnosis:
Presumed schizophrenia, delusional disorder, sleep disorders (insomnia, sleep paralysis, night terrors, sleep walking), sexual deviances (observed homosexual tendencies), hysterical behaviour.

POLICE/INCIDENT REPORT

Mode of Admission:
Involuntary commitment — Patient resided in a Catholic convent where several nuns witnessed her erratic behaviour and presence episodes of delusion that made the institution decide the patient was dangerous. Upon conformation from the psychiatrist team of the Trinity Hospital patient was committed to The Van Kovak Institute of Behavioural Health Sciences.

Mental State on Admission:
Patient denied needing commitment into the institution, apparently seeming to believe the priests who brought her in had conspired to make her ‘look crazy’. At first patient acted calm and peaceful but upon realising she’d be staying in the institution for the foreseeable future, her behaviour became hysterical and sedation was needed.

Physical Condition on Admission:
Patient presents various bruises in different areas of the body plus some likely older scars across her upper back.

Doctor’s Notes:
Patient seems convinced she is in no need of medical attention and denies being delusional. She is capable of reasoning and does not present many of the symptoms and behavioural issues commonly seen in a schizophrenic profile, so her diagnosis is still quite uncertain. Miss Blackmore has confirmed to several doctors that her hallucinations are vivid although she does not believe them to be products of her sickness. She does not present many behavioural issues and contrary to what is expected she presents a non-confrontational type of personality. She only seems to display an aggressive behaviour towards her hallucinations. One of the most concerning trait about Miss Blackmore so far is her incapability to realise that her disorders are such, thus denying needing any mental healthcare.

Intense rage issues, defiance disorder, borderline personality disorder, manic depressive, hyper sexual.

Addictions:
Sex, drugs, smoking, fighting, defiance.

Self-medicated (until hospital stays)

POLICE/INCIDENT REPORT

Mode of Admission:
Criminally- The patient was brought in by no less than a dozen bloodied officers. The patient was resisting arrest, screaming obscenities, and the moment the officers turned away from him, he was attacking them once again. Upon further inspection, a bloodied ear was discovered in patients mouth, which was spit out at the first doctor to approach him.

Mental State on Admission:
Patient was in a complete rage, unreasonable, incredibly strong, and aggressive.

Physical Condition on Admission:
The patient had broken handcuffs, as he had snapped them prior to arrival, and was covered in blood that wasn’t completely his own. His clothes were bloody, but he was physically fine, save for a bruise or two.

Doctor’s Notes:
“Unpredictable, highly dangerous, oddly stable. It is recommended to keep this patient drugged, otherwise the chance of a rage becomes imminent. He pretends to be charming to secure what he wants, but cares none for emotional or the mental or physical well being of himself or others. He insists he is not insane, and refuses any type of regular medicine.”
